Nursing Care Coordinator

Salary: $35.00 to $44.00 hourly

Summary: Work within the scope of a Registered Nurse (RN) and Certified Diabetes Educator (CDE) to assess, plan and implement diabetes and chronic disease related self-management. Work collaboratively with Diabetes Wellness Team (DWT) members as required. Contribute to a shared care model. Work with relative autonomy utilizing medical directives specific to the DWT. Provide self-management education to clients, families and communities. Duties: 1. Provide diabetes self-management education § Providing education for individuals, family, community health promotion and chronic illness prevention 2. Implement and monitor client care plans§ Complete health assessments, plan, implementation, and evaluation of individual care plans to promote self-management of diabetes and chronic illness§ Work alongside the DWT Registered Dietitian to provide a comprehensive health assessment§ Review lab values related to chronic disease management§ Complete medication reconciliations and provide education
