Case Management Community Outreach Response Team (CORT) CORT is a mobile, rapid-response team that provides intensive, wrap-around support to people living with HIV who are also experiencing addictions, mental health concerns, and other psychosocial challenges. The goal is to improve health and social outcomes through quick intervention and coordinated case management. This is a temporary contract to March 31, 2027, with potential for renewal. Temporary position until March 31, 2027 with an opportunity to extend.
