Job Summary: We are seeking a dedicated and compassionate Tawâw Case Manager to join our dynamic team. In this vital role, the Case Manager supports individuals accessing services through the Tawâw Permanent Supportive Housing program by providing culturally informed, client-centered case management. The role focuses on building trust, assessing needs, coordinating supports, and helping clients achieve stability in areas such as housing, health, income, and wellness.. *Duties*:. Conduct intake assessments and develop individualized service/care plans. Provide ongoing case management and regular follow-up with residents. Support residents in accessing income supports, healthcare, mental health, and addiction services. Advocate on behalf of residents with community agencies, and resources. Maintain accurate and confidential case notes, reports, and documentation. Identify barriers to stability and collaborate on practical solutions. Crisis intervention and de-escalation when required.
