Reporting directly to the Home Care Supervisor, this position is part of a multidisciplinary team that provides client focused, family centered care to individual's families and communities of BRT6HC. The Chronic Conditions Nurse (CCN) is responsible to provide expertise education and guidance to the Chronic Conditions Team, work with the team to establish care strategies for chronic diseases that are prevalent in the community, plan and develop, as well as implement and di
