Overview: The Stratford Family Health Team (SFHT) is seeking a dedicated and collaborative Nurse Practitioner (NP) to join our interdisciplinary care team. The success full candidate will be employed by the Stratford Family Health Team and assigned to work within the Mitchell Family Health Organization (MFHO) clinic in Mitchell, Ontario, providing primary care services in support of MFHO physicians and their patients.. Working closely with primary care physicians, clinic staff, and team members, the NP will provide comprehensive, patient-centered care to patients rostered to the MFHO physicians. The successful candidate will assess, diagnose, treat, and manage a wide range of acute and chronic health conditions, promote health and disease prevention, and contribute to improved patient outcomes across the lifespan. Responsibilities: Provide comprehensive primary care to patients rostered to MFHO physicians. ordering and interpreting tests, prescribing medications, and developing and imp
