Key Responsibilities: Case Management, guided by the company's Client Pathways service model, which encompasses professional resources management, goal setting, team management, case budgeting, and third party funding approvals. Complete hospital discharge plans. Setting rehabilitation goals in consultation with clients, their families, case teams, and other treating professionals.
