Reporting directly to the Sohkayawin Housing Manager, the Intensive Case Coordinator is responsible for delivering housing stabilization and intensive case management services to tenants within the apartment complex. This position involves working in a high-risk environment with individuals experiencing complex challenges, including mental health concerns, substance use, and histories of trauma. The Intensive Case Coordinator works with medium- to high-acuity tenants, providing a holistic, trauma-informed, and culturally responsive approach to care. This position also plays a key role in coordinating team efforts, and ensuring tenants maintain stable housing and improved overall wellbeing.Job Expectations: Provide intensive, tenant-centered case management to individuals living in permanent supportive housing.
