Duties include:-Coordinate and facilitate individual and group life enrichment activities in personal homes and in the community, enhancing the quality of life for individuals living with dementia. - Coordinate and facilitate supportive living services to clients to support health aging at home.- Build trusting relationships with care partners to understand the needs of their loved ones and how to best support their quality of life through person-centered planning.- Work collaboratively with other team members to ensure the needs of the clients and care
