The Worker will assist and support individuals to make healthy choices that maximize their potential and quality of life. Support work will involve finding safe and appropriate housing, helping with personal care, cleaning and maintenance of the home, budgeting and managing finances, providing transportation in and outside of the community, accessing health and community services, attending events in the community , participating in social and/or recreational activities. Scheduled work times, Evening and weekend work expected. Key Responsibilities- Work one on one with individuals and their families/supports to determine which areas of life can be enhanced. Meet with individuals on scheduled time and provide support and assistance to accomplish whatever goal/activity is planned for the day. Teach daily living skills and track progress.
