Responsibilities: Conduct Occupational Therapy In-Home Assessments to assess Attendant Care Needs (Form 1) and assistive devices Provide treatment with Implementation of functional and cognitive strategies and techniques as appropriate Demonstrate Diversity, Equity and Inclusion principles with all clients and family members Document accident benefits initial and progress reports Supervise rehab support worker with carry through of Occupational Therapy goals Provide education to clients/family; follow-up with progress with respect to SMART goals Work collaboratively with an interdisciplinary team and participate in meetings Facilitate rehabilitation of client with cognitive, emotional or physical impairments Monitor and assess client rehabilitative programs Convey treatment results and client p
