They provide guidance and support regarding various aspects of creating an SCC plan and work with all SCC stakeholders to ensure the process is followed including completing necessary forms, providing documentation, accessing expert advice, and facilitating the development of a comprehensive community treatment plan that addresses each individual's treatment and support needs.
