NURSE PRACTITIONER ? Corunna - Full Time (35 Hrs/week): The *Thamesview Family Health Team (FHT) *provides primary health care, including chronic disease management and health promotion and disease prevention to a rostered population of patients in the Chatham-Kent and Sarnia - Lambton. The FHT utilizes an interdisciplinary team of health care professionals who work in a collaborative model of care.. The Nurse Practitioner (NP) will establish a practice, and enroll a group of patients and operate as the primary care provider. This Position is designed as an integrated physician-NP model where the NP works in close partnership with family physicians. The successful candidate will maintain their own clinical expertise while participating in shared care, regular case discussions, and collaborative care planning.
