Nurse Practitioner

Position Purpose: The Nurse Practitioner provides non-urgent primary care assessments in person or by telemedicine to clients. The position promotes continuity and effective transitions for clients and families and addresses their ongoing needs. The role involves collaborative leadership, formal education sessions, and coaching to enhance the provision of quality care Hours of Work and Scheduling: This remote Nurse Practitioner position offers up to 30 hours per week, generally during daytime hours, Monday to Friday. The schedule will ideally be spread across three to four days per week. Four days of coverage per month are required: Wednesdays (admission day) and Thursdays (the following day) for the admission cycles at both residential houses. Remaining hours will be scheduled collaboratively based on program needs and the Nurse Practitioner?s availability Responsibilities: Service.
