Position responsibilities: Conduct psychosocial assessments and identify patients? social and emotional needs. Provide individual counselling and supportive interventions. Support patients experiencing stress, anxiety, grief, family concerns, and other psychosocial challenges. Assist patients with navigating community resources and social services. Provide support with housing, financial, employment, and other social needs where appropriate. Develop individualized care plans and provide appropriate follow-up. Collaborate with physicians and other healthcare professionals as part of a multidisciplinary team. Provide crisis support and appropriate referrals
